Please correct the following before submitting:
    New/Returning to WSYSL*
    Coaches
    Add up to 6 coaches total. Check "Co-Head Coach" for up to one additional coach (2 head coaches max, including the Head Coach below). Phone and email are required for any coach you enter.
    If Part of a larger Organization
    Which division do you think your team should play in?*
    (The final decision will be made by the league coordinator.)

    Checks should be made out to the "WSYSL"
    Registration form must be accompanied by the $200 League Fee.
    There will be no refund of League Fees once a team has been accepted into the WSYSL.
    Once you submit the form, you will be immedialty prompted to print the page. Please print the page and send it in along with your League Fees.
